Kade was born with Hypoplastic Left Heart Syndrome, meaning only half of his heart worked properly.
The 1-year-old has spent most of his young life in the hospital, waiting for a transplant but in March, the family revived their life-changing phone call, little Kade now had a heart.
"Everybody is screaming and crying," said Kade's mother, Kimberli Gravely.
Since the transplant, Kade has been doing well, but the moment is bittersweet.
"It's a lot. There are a lot of different emotions. We are very blessed and thankful for this to have occurred and for him to have gotten his new heart, but at the same time, you think about the other family and wonder what they have been going through," Kimberli said.
On Kade's first birthday, the family threw him a "Moana"-themed party. Kade was dressed as his favorite character, Maui.
"The whole time he was in the hospital he loved the movie 'Moana.' I don't know if it was the singing or the colors. So, we decided that was going to be the theme for his first birthday," Kimberli said.
Kade's little sister even sang him a song from the popular Disney movie.
There was a table full of gifts, but for the family, the best gift of all is having their son alive and well.
"You thank the good Lord above for this miracle because he truly is a gift from God," Kade's father Joe said.
The family wants to stress the importance of organ donation.
"Be an organ donor. Organ donation is life," Kimberli said. "That's what saved Kade was one decision of somebody making their child an organ donor and that one decision saved his life."
